Responsible to: Pharmacy Manager
Job Summary
The role involves drug chart clinical screening, medicines reconciliation and medicines optimisation, patient counselling, and contributing to medication safety audits. You will work as an integral part of the wider clinical team, delivering a high standard of efficient, patient-centred care. You will be expected to adhere to St Joseph’s Hospital policies and procedures to support the best possible clinical outcomes for our patients.
Key Responsibilities
• To deputise for the Pharmacist/ Pharmacy manager in their absence carrying out all professional pharmaceutical responsibilities.
• To provide clinical pharmacy services and medicines to wards, theatre, outpatients and other hospital departments.
• To apply medicines reconciliation when checking inpatient medication, clinically screen drug charts and all inpatient and outpatient prescriptions.
• To provide high quality care across a range of specialties.
• To aid seamless pharmaceutical care across the various healthcare settings.
• To provide medicines information to all healthcare teams/ professionals.
• To ensure all medicines procured are safely received and stored in pharmacy.
• To ensure that when medication errors are identified, they are recorded, reported and handled appropriately.
Person Specification

Essential
• Master’s degree in pharmacy or equivalent
• Registration with the General Pharmaceutical Council of Great Britain
• Fulfilment of mandatory Continuous Professional Development requirements
• Good verbal and written communication
• Clinical knowledge of current therapeutics and pharmacy practice
• Flexible and adaptable approach
• Commitment to undertake personal development planning
Desirable
• Diploma in Clinical Pharmacy
• Experience of a hospital pharmacy environment
